EJHS will be transferring to Final Forms for all athletic physicals – We partnered with Final Forms (see letter) to create a better system in handling athletic physicals and creating a student safety tool for our coaches, athletic trainers, and athletic administrators. For 2023-2024 all athletes will be required to have a Final Forms account. This replaces the need for you to fill out a physical on paper as the pre-physical checklist can be completed online with an autofill for information based on your responses. 

– If you HAVE Final Forms, you should sign in and update your information for your athlete.
