2025 Athletic Physical Night

1) Registration – The cost of the physical is $25 plus transaction fee and can be purchased through EventLink to complete your registration. We are only taking payment via EventLink for this event so paying ahead of time is important.

Select “2025 Physical Night (April 9th, 2025)”.
Select one of three time slots. There is a cap on the number of physicals we can process in an evening, when we hit our maximum capacity for that time slot, it will not be available.

2) Final Forms – We have partnered with Final Forms (see letter) to create a better system in handling athletic physicals and creating a student safety tool for our coaches, athletic trainers, and athletic administrators. All athletes will be required to have a Final Forms account. Please use your student’s school issued email when creating your student’s account. This is how the coaches and school officials will communiate with your athlete about importation information.

So what do I need to do before the EHS Physicals Night?
– If you HAVE Final Forms, you should sign in and update your information for your athlete.
– If you DO NOT have Final Forms, follow the Parent Playbook to set up your account.
(5th Grade Parents: We are aware FinalForms will not allow you to select 5th grade. We ask that you do one of the two things: 1 – select 6th grade and the office will update your students account to indicate the correct grade. OR 2 – Send a copy of your student’s physical to Mrs. Norris at jnorris@rbbschools.net if you plan to go somewhere else to complete the physical. We will then notify you when FinalForms is available for registraion.)
